Many catastrophically injured clients and their families struggle in the community with little to no assistance.  Following an incident/accident, resources become drained quickly. Often a spouse quits a career to stay home and care for the injured/infirm, and little to no community resources appear to be  available. A Nurse Case Manager is skilled at researching and accessing seemingly hidden resources.

  1. Is there great difficulty completing activities of daily living such as bathing, grooming, toileting, and meal preparation?
  2. Is there concern short term memory loss or cognitive decline could be causing a risk for injury or harm?
  3. Is leaving home a taxing effort especially for visits to medical appointments, grocery shopping, and visits to pharmacy or other necessary errands?
  4. Is it difficult to walk short distances without becoming short of breath and/or causing exhaustion?
  5. Does the individual attend events such as weekly church gatherings, reunions, weddings, and family gatherings like as they were accustomed or do they avoid them due to cognitive decline, decreased functional level or decreased energy reserves?

There are 5 questions anyone may ask about a client, family member or friend to ascertain whether they might benefit from unskilled home care services.  Resources are often limited for needed care and reliance on family members becomes necessary.  A home care rider in a long-term care insurance policy could be a resource, however this is not typical for the average family.
